Please fill out the initial application below. A staff member will contact you by phone within twenty four hours to talk about your goals.
An application form detailing certain requirements that must be met, including:
- Furnishing a copy of your driving record
- Furnishing a copy of your criminal record (if applicable)
- Being able to obtain an FAA medical certificate
will be mailed, emailed, faxed or available on the website (you choose).
This is an initial application. Submit this form and an MPD staff member will contact you within 24 hours.
